SMWJFC Track & Trace

SMWJFC are committed to ensure all involved with and whom visit the football club venue stay safe from COVID-19 and in compliance with the Governments Track and Trace system we ask that you complete this form prior to or upon entering the venue. If an instance of COVID develops from a visitor to the club then all the information of all persons who may have come into contact with the person showing symptoms shall be notified to the government track and trace. All data shall be held for 21 days and then deleted.
